The Moon government 's health care policy emphasizes primary care and public health. In this article, we will look at the definition of community based primary care and public health and try to identify the meaning of community based primary care projects. Community based primary care projects is a project to establish an effective chronic disease management system linking local community resources to outpatient clinics. The establishment of this project based on the payments system to enhancing the redefining functions of primary care in the healthcare system. Another ultimate goal of this project is to make 'primary care platform', for integration care including the welfare resource linkage. This will contribute to improving the quality of life across health and welfare in terms of community approach to health problems.
